Can you reproduce the crash? What is the error msg, etc?

What do you mean with reversed? It looks like it did refine into the
density jump.

What settings do you use for refinement, something like this?
  set Refinement fraction                = 0.3
  set Coarsening fraction                = 0.05
  set Time steps between mesh refinement = 5
