Chris- I think you have encountered a known bug in creating cohesive cells. We have found some bugs with creating cohesive cells for 3D meshes. Matt has been working on it but we don't yet have a solution. Cohesive cells for hex8 meshes are less error prone than those for tet4 cells. Brad
