It looks like you are using quadrature for tet cells with a hex mesh. 
The number of edges in the reference cell (from quadrature) is 3 while 
the cell in the mesh has 4 edges. You need to change the quadrature 
settings. See examples/3d/hex8 for how to setup the quadrature for a hex 
mesh.
