[CIG-SHORT] 2D Simulations

PyLith does not support 2-D out-of-plane deformation. We have not
implemented this capability because it involves writing extra code to
handle this special case. The workaround is to construct a 3-D model and
apply Dirichlet boundary conditions to enforce the desired symmetry and
extract the solution along the centerline. This is more computational
intensive but works reasonably well.
