[CIG-SHORT] Fault junctions

Ali,

The current limitation in PyLith is that faults cannot share
nodes/vertices. This means that if you have a main fault and a fault
branch, the branch must end 1 cell away from the main fault.
Additionally, this node should have zero slip to avoid effects
associated with slip at the end of the fault.

In dynamic rupture benchmark tests, we have found relatively small
differences between this implementation and other codes that are able to
allow slip on the node 1 cell away. By selecting the "main" fault as the
one that has the more continuous distribution of slip you can minimize
errors associated with not having an ideal intersection.
