[CIG-SHORT] Strange artifact on edge of 3d simulation

Hi everyone,

I was running a simulation which extrude the 2D mesh of 
example/2d/subduction to 3D, and with the help of Brad that simulation 
went through well. But when I start to use a refined mesh, strange 
artifacts show on one of the edge of the model which can be seen from 
the gif animation. The screenshot is the wireframe wrapped by 
deformation in x-y direction. I suspect that this is because the mesh 
quality is poor there, but if this were the case, similar artifacts 
should have also appeared on the other side. All files are attached.
