Read through the manual and find examples that are most similar to what 
you want to do. Change them piece by piece so that they get closer to 
your problem. You may also want to look at the dyanmic spontaneous 
rupture benchmarks (although they are not all updated for the latest 
version of PyLith) which are in the pylith_benchmarks repository 
(https://github.com/geodynamics/pylith_benchmarks).

You may be able to use Trelis from csimsoft. This is for academic and 
other use.

You would need to convert the mesh to the ASCII format PyLith uses in 
the trivial examples or the preferred method of the ExodusII form of 
NetCDF 
(https://cubit.sandia.gov/public/13.2/help_manual/WebHelp/finite_element_model/exodus/exodus2_file_specification.htm).
