[CIG-SHORT] Geofest trial input - Reverse Slip, variable resolution...

I tried to make a geofest input file for Carl's variable resolution mesh (no
smoothing) #01, and the input deck looks good to me (again, ~23,000 nodes
makes it difficult to check by hand). However, I get an error when I try to
run it with GeoFEST 4.5

An error occurred:
IAM: 0    error code = 43


I get the same error using the bmrsnog_tet4_1000m.gft file for the uniform
mesh that I got from the CIG site, which is not surprising since I based the
input file off of that. If someone in the know in the geofest community can
take a look at the input file at
http://www.gps.caltech.edu/~eah/CFEM/tet_var_res_01.gft.gz
and give me a hint as to where the problem is, I would appreciate it.

Although maybe this is a moot point, since I am not sure if at this point in
time further work will be included into Brad's SCEC poster...
