Xu,

Using the journal files that you sent with a discretization size of 80 m 
on the fault, the maximum condition number is about 1.5, which is a high 
quality mesh. However, you do use a bias factor of 1.07. Using something 
around 1.02-1.04 will give you a smoother gradation in cell size.
